Summary:

As a part of the Goldbelt Glacier Team, you will be supporting the Naval Logistics Command with medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW), you will be assigned to either a Military Treatment Facility (MTF) or other Federal medical treatment facilities in Japan.

Responsibilities

Essential Job Functions:

  • Assess each patient and perform triage duties as assigned.
  • Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
  • Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.
  • Provide treatment and discharge instructions upon patient release.
  • Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
  • Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
  • Assist in planning, provide clinical direction, and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel.
  • Demonstrate sound knowledge base and action in the care and decision making for designated patient populations. If weaknesses or deficiencies are identified either by peer reviews, quality control reports (QCRs), or evaluations the HCW will be required to complete refresher training (in-services, competency reviews, continuing education, etc.) at contractor expense.
  • Perform documentation (manually and/or by use of computer) duties on unit and in-patient record that are timely, accurate, and concise.
  • Establish priorities of patient care based on essential patient needs and available unit, command, and community resources including time, personnel, equipment, and supplies.
  • Demonstrate competency in medication and intravenous (IV) administration to include blood products and Total Parental Nutrition (TPN) intramuscular (IM) and subcutaneous (SQ) administration and demonstrate competency in phlebotomy as specific to the position.
  • Safely administer prescribed medications to include intravenous drips and intravenous push drugs.
  • Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.
  • Perform documentation (manually and/or by use of computer) duties on unit and in-patient record which are timely, accurate, and concise.
  • Assist with the teaching of formal and informal classes, as directed by the Government Supervisor, to new paraprofessional personnel with the goal of helping them successfully complete competency-based skills list and weekly goal setting, assisting in the
  • Demonstrate effective and professional communication methods, and skills, using lines of authority appropriately.
  • Formulate and use effective working relationships with all health care team members, patients, and significant others. 
  • Remain flexible in staffing patterns and resolution of staffing conflicts; participate in temporary assignment measures.
  • Provide individual, group, and family/significant other counseling and health teaching in relation to the patient's condition and his/her ability for self-care. Provide for accessibility to various literature resources dealing with subjects related to specific patient health
  • problems. Provide physical and psychological support to patients and significant others, explain procedures and treatments, and promote cooperation among staff, patients, and significant others.
  • Assist in the orientation and training of newly assigned unit personnel as directed by Government Supervisor.
  • Provide input into competency assessments of newly assigned unit personnel as directed by government supervisor.
  • Collaborate in the review of clinic standing operating procedures as directed by government supervisor.
  • If directed, perform phone triage based on accepted protocols and assist as needed in accessing appropriate ambulatory/emergency care for beneficiaries.
  • Provide training and/or direction as directed by the government supervisor to supporting government employees (i.e. hospital corpsmen, technicians, students) assigned to the HCW during the performance of clinical procedures. Such direction and interaction will adhere to government and professional clinical standards and accepted clinical protocols.
  • Participate in executing the Emergency Preparedness Plan (drills and actual emergencies) as scheduled by the MTF (typically semiannually).
  • Participate in peer review and performance improvement activities.
  • Participate in the implementation of the Family Advocacy Program as directed by government supervisor.
  • Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting.
  • Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF, disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical,
  • Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.
  • Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.
  • Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions.
  • Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage.
  • Comply with the standards of The Joint Commission, applicable provisions of law, and the rules and regulations of any and all governmental authorities pertaining to licensure and regulation of health care personnel and medical treatment facilities, the regulations and standards of medical practice and the MTF’s medical staff bylaws.
  • Participate in continuing education to meet own professional growth.
  • Participate in the provision of in-service training to clinic staff members.
  • Provide training and/or direction as applicable to supporting government employees (e.g., hospital corpsmen, students, etc.).
  • Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander.
  • Attend and participate in various meetings as directed by government supervisor.
  • Shall comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility. Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and
  • Perform timely, accurate, and concise documentation of patient care.
  • Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.
  • Provide consultation from a nursing standpoint in complex cases and act as a point of contact for
    professional colleagues involved with patient and patient’s family for the following clinics Family Health, Women’s Health and/or Pediatric Clinics. The RN will assist in preparation of histories and clinical records, ensure continuity of care of referrals under provider’s guidance, and ensure follow up appointments are made in a timely manner for patients after referrals are completed.
  • Assist in preparation and review of case histories and clinical records as needed. Under the provider’s
    guidance, the RN will order diagnostic procedures and imaging studies including radiographs, MRI, CT scans, clinical laboratory tests and other diagnostic imaging studies as appropriate. The RN will ensure providers are updated of all imaging and lab results.
  • Actively participate in ongoing improvement projects, depending on clinical needs, associated with disease management, prevention, and population-based health management. Ensures initial and ongoing assessments identify patient needs, issues, resources and care plans in a timely manner including patient health education needs.
  • Assist the HCI and providers to re-assess and refine clinic level prevention programs. Assist with
    implementation of new or changing prevention recommendations for the clinic team. Provide disease
    management/population health focused in services to team.
  • Educate and counsel at appropriate age levels patient and patients’ family about their potential risk for
    diseases or injuries, using clinic approved literature or websites. The RN will ensure the patient/family’s
    understanding and compliance and make appropriate recommendations and referrals.
  • Triage questions regarding medical necessity, appropriateness of care, and quality concerns for patients using established guidelines from telephone triage protocols. Use appropriate resources and provider for consult management. These telephone consults (t-cons) will be generated through AHLTA. Each t-con will evaluate patient needs and respond to identified needs and/or redirect concerns to appropriate level of care. The RN will ensure symptom based t-cons are addressed within 24hours and non-symptom based t-cons will be addressed within 72 business hours. If there are problems with computer access, these issues must be clearly documented.
  • Ensures coordination of care delivery processes to enhance patient’s health and wellness, safety,
    productivity, and quality of life, to include alternate care settings and the home environment.
  • Demonstrate strong customer service skills and foster development of those skills throughout entire PCM team.
  • Furnish procedures compatible with the medical facility's operating capacity and equipment. New medical procedures/services shall not be introduced without prior recommendation to, and approval of, the MTF Commander
    or authorized representative. The RN should be proficient in or willing to train on the following procedures: IV placement and medication administration, IM medication administration, oral mediation administration, physical assessments, vision assessments, optoacoustic emissions test, nebulizer treatments, vaccination administration, ear irrigation, venipuncture, bladder catheterizations, Pulmonary Function Tests.
  • Histories and exams performed by the RN will be pertinent and clearly documented in a timely fashion in AHLTA or electronic health record. Documentation must be clear, concise and complete for all patients. Nursing
    diagnosis will be accurate with appropriate action and follow-up. Referrals will be appropriate for diagnosis and
    assessment and age appropriate preventive screenings will be demonstrated.
  • Document evaluation of pain, medication administration and re-assessment of pain. Medication reconciliation will
    be documented for all encounters. Correctly use and document Clinical Practice Guidelines (CPGs) and Staff Support
    Protocols (SSP) per facility standards. Follow up abnormal vital signs and document actions. All clinic procedures
    will be documented accurately and in a timely manner.
  • Assist the HCI and providers to re-assess and refine clinic level prevention programs. Assist with
    implementation of new or changing prevention recommendations for the clinic team. Provide disease
    management/population health focused in services to team.
  • Support providers on transport of patients on an as needed basis during regularly scheduled hours. 
  • Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required.

Qualifications

Necessary Skills and Knowledge:

  • Ability to maintain a level of productivity comparable with that of other individuals performing similar services.
  • Ability to adhere to and comply with the Department of the Navy, Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.
  • Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.
  • Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies.
  • Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook.
  • Knowledge, Skills, and Abilities. Knowledge of professional care theories, principles, practices, and
    procedures. Experience in assessing pediatric, adolescent, adults, obstetrics, and geriatric patients, as appropriate.
  •  Knowledge of a wide range of medical disorders/conditions and disease processes across the lifespan, as appropriate. Knowledge of preventive health schedules, procedures, processes and education methods as well as knowledge of disease management and population health principles.
  • Knowledge of a variety of pharmacologic agents used in patient treatment, the desired effects, and
    complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculation as required. Knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, acknowledgment of teaching, and follow up care.

Minimum Qualifications:

 

  • WOMEN’S HEALTH NURSE REQUIREMENTS: The RN will focus on offering primary care services to
    women of all ages. The RN shall provide care for acute and chronic health problems, but they are also skilled in offering health counseling services as well as preventative services, and referrals to relevant specialists when necessary. The RN shall generally work with patients beginning at puberty through their advanced years, providing health care, disease prevention, and health promotion services. The RN shall provide a full range of Gynecological and Obstetrical services in accordance with privileges granted by the Commanding Officer (e.g. provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; immunizations; diagnose, treat, and counsel patients as indicated).
  • Education. Must have graduated from a baccalaureate degree program in nursing (BSN) accredited by a national nursing accrediting agency recognized by the US Department of Education.
  • License/certification. Nurse applicants must be a current U.S. licensed Registered Nurse. Active, current, valid and unrestricted license to practice nursing in accordance with State Board requirements. License cannot beunder investigation nor have any adverse action pending from a Nursing State Board or national licensing/certification agency.
  • Experience - RN must have at least two (2) years of clinical experience in a primary care setting to include; outpatient clinic, inpatient hospital, emergency room or urgent care.
  • Possess and maintain a current unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands
  • HCW shall be in good standing, and under no restrictions, with the state licensure board in any state in which a license is held or has been held within the last 10 years.
  • HCW shall be in good standing and under no sanction or suspension by the Federal Government.
  • Possess and maintain current certification in either the American Heart Association BLS for Healthcare Providers or American Heart Association Healthcare Provider Course.
  • Able to read, write, speak, and understand the English language fluently and maintain good communication skills with patients and other health care personnel.
  • Ability to pass all background/suitability checks.
  • A valid driver’s license; ability to travel locally as needed.

  • Highly developed written, oral, and presentation communication skills
